Tourniquet
Tourniquet Use and AKI /
The other day, I had a referral for preop assessment for a surgery that potentially could have a tourniquet applied. The problem was that he probably had some CKD and AKI on top of it. When I reviewed him, I suggested that tourniquet application may not be suitable in view of possible reperfusion injury. But it was an educated opinion at best at the time.
I may be right, but the reasoning is relatively wrong. It's probably more from the rhabdomyolysis than the reperfusion with systemic inflammation.
Perioperative Tourniquet Use in Sickle Cell Anemia /
Interesting take, saying that it is safe.
Until it is rigorously tested by RCT's, it is difficult to make a conclusion.
The more we know, the more we don't know.
